What this template includes
The main table has six columns: Medication (the drug name), Dose (e.g. 10 mg), When Taken (e.g. once daily with breakfast), Prescriber (the doctor or provider who prescribed it), Pharmacy, and Started (the date you began taking it). The optional emergency block at the top of the sheet has fields for an emergency contact name, phone number, relationship, and a box for known allergies.
How to use this medication list
- Choose how many rows you need — the default is 12, which fits one page neatly. Increase to 15 or 20 if you take more medications.
- Turn the emergency block on or off — if you want the emergency contact and allergy section at the top, leave it on; if you want more space for medications, turn it off.
- Print the sheet — click Print or press Ctrl+P (⌘+P on Mac).
- Fill it in carefully — write each medication exactly as it appears on the label, including the dose and instructions.
- Update it when anything changes — print a fresh copy whenever a medication is added, stopped, or the dose changes.
Practical tips
- Keep a copy on the fridge, in your wallet, and in your medical bag — so you always have an accurate list available in any situation.
- Bring the sheet to every medical appointment — having a complete written list saves time and helps prevent errors when a new provider reviews your care.
